Instructions: Please complete
this form and fax to 973-761-8938, Attention:
Education Dept. Notebooks take 7-10
business days for delivery. Upon
receipt of this order form you will
receive a phone call to confirm that
your order has been placed. Delivery
must go to your home address and
not to the site of the seminar. NCCDP
is not responsible for any shipments
that do not arrive in time for your
seminar. This order is non-refundable.
It is the instructor’s responsibility
to place the order in enough time
for delivery to take place. Only
NCCDP approved instructors are authorized
to order and use the notebooks for
teaching purposes only.
